HM Yellow Dot Plus
The Yellow Dot+ Program is a voluntary Hershey’s Mill (HM) program designed to help residents secure and organize important personal health information in a consistent way that facilitates effective and efficient care during a medical emergency. HM Security answers approximately 300 medical-related calls annually. They have found that the information provided by residents in the Yellow Dot packets has helped emergency medical providers (EMS) and the HM Security better care for ill or impaired residents. This level of preparation is particularly important when residents need immediate care, but are alone, unresponsive, or incoherent, and/or neither they nor family members can provide important health information quickly. In emergency medical situations, the more quickly EMS can provide correct care, the better the outcomes.

- Then, push the emergency button in your home or call HM Security at(610 436-6403).

- Medical Information packet that includes standard forms that (when completed and updated by the resident) should be placed on the resident’s refrigerator in the easily identifiable Yellow Dot+ envelope.
- PennDOT Yellow Dot Pamphlet to record important relevant medical/personal information and universally recognized sticker that is used in your vehicle. This Yellow Dot Pamphlet and sticker are now recognized by EMS providers in 22 states, and there is proposed legislation in many other states to institute the program.
- General instructions on how to program your cell phone to provide emergency contact information that could be accessed in the event of an emergency.

- New residents should check with their Village President to request a YD+ Packet.
- All residents may pick up YD+ Packets and/or extra PennDOT Yellow Dot Pamphlets in the HM Library or Front Lobby of the Community Center. Limited numbers may be available at the HMCTV Office.
