HM Yellow Dot Plus

The Yellow Dot+ Program is a voluntary Hershey’s Mill (HM) program designed to help residents secure and organize important personal health information in a consistent way that facilitates effective and efficient care during a medical emergency. HM Security answers approximately 300 medical-related calls annually. They have found that the information provided by residents in the Yellow Dot+ packets has helped emergency medical providers (EMS) and HM Security better care for ill or impaired residents. This level of preparation is particularly important when residents need immediate care but are alone, unresponsive, or incoherent, and neither they nor family members can provide important health information quickly. In emergency medical situations, the more quickly EMS can provide correct care, the better the outcomes.

In the Event of a Medical Emergency

  1. Call 911 (EMS) first.
  2. Then, push the emergency button in your home or call HM Security at (610) 436-6403.

NOTE: Calling HM Security first will delay the call to EMS and will delay EMS’s response to address your health and medical needs.

The Yellow Dot+ Program Provides HM Residents With

  1. Medical Information Packet that includes standard forms that, when completed and updated by the resident, should be placed on the resident’s refrigerator in the easily identifiable Yellow Dot+ envelope.
  2. PennDOT Yellow Dot Pamphlet to record important medical and personal information, along with a universally recognized sticker for use in your vehicle. The Yellow Dot Program is recognized by EMS providers in 22 states, and legislation has been proposed in many other states to implement similar programs.
  3. General Instructions for Your Cell Phone to provide emergency contact information that can be accessed in the event of an emergency.

How to Get Your HM Yellow Dot+ Packet

  1. New residents should contact their Village President to request a Yellow Dot+ Packet.
  2. All residents may pick up Yellow Dot+ Packets and/or extra PennDOT Yellow Dot Pamphlets in the HM Library or the Community Center Front Lobby. Limited quantities may also be available at the HMCTV Office.

A fillable PDF for your medical information is available if you do not wish to use the pre-printed form provided, or if you need to update your information. Download the form to your computer, enter your information, and print it. The completed form may be placed in your Yellow Dot+ Packet on your refrigerator in place of the blank form included in the packet.
